different types of general dentistry services: 120,000 rials

Annual membership fee for using 70% of allowance for different types
of general dentistry services: 180,000 rials

Annual membership fee for using 80% of allowance for different types
of general dentistry services: 300,000 rials (if fulfilling the household
condition)

Annual membership fee for using 90% of allowance for different types
of general dentistry services: 420,000 rials (if fulfilling the household
condition)

Membership fee for personnel of organizations and companies on a monthly
basis in the amount of 10,000 rials for 60%, 15,000 rials for 70%,
25,000 rials for 80% and 35,000 rials for 90%

The entire annual membership fee for individual applicants is received
upon enrollment.

To use dentistry allowance, it is necessary to provide the membership
card.

Because of its confidence about the quality of its services and its
respect for you, if you are dissatisfied with the services provided,
you can replace or return your membership card within one month of
the membership even if having used any service, and you will be refunded
without any deduction and with full respect.

Use of all the membership benefits is open to the public without any
limitation as to age, job, etc, individually or collectively, (until
further notice).

To use a surgery allowance or interest-free loan, it is necessary
to receive a letter of recommendation from the head office of the
company prior to surgery.
Documents needed and centers for enrollment of members:

1 photo, copies of the first and second pages of ID card, receipt
of payment of membership fee.

Membership applicants may deposit their membership fee to the current
account of Shamim Kowsar Treatment Services Consulting Cooperative
Company. It is evident that the company will have no responsibility
for payments to an account other than the account of Shamim Kowsar
Co.

enroll at the head office and authorized representative offices as
well as medical centers under contract, and receive the membership
